Job summary

We have two fabulous opportunities for District Nurse / Community Nursing Sisters/Charge Nurse to join Derby City Community Nursing.

We currently have two vacancies:

Wilson Street Team - who provide nursing care for both Wilson Street Surgery and also Derwent Medical Practice. This post is for an immediate start - based at Coleman Health Centre.

Macklin Street Surgery - providing nursing care for patients registered at Macklin street surgery. This post is to start in September - based at Village Health Centre.

Both teams provide nursing care across derby City offering a varied and vibrant demographic

This is an ideal opportunity for a registered nurse with varied community experience who wants to develop their leadership and caseload management skills in a supportive environment or a District Nurse or community sister/charge nurse who is ready for a change.

Main duties of the job

The successful applicant would require community nursing experience, some knowledge of caseload management with a high standard of community nursing knowledge and clinical skills.

You will be provided with training and support tailored to your needs to drive forward high quality care and team work; including a thorough induction programme, regular one to ones, peer supervision, working within an MDT and clinical training.

Provide effective evidenced based care to a defined population/Primary Care Network. This is achieved through effective delegation, case load management, monitoring of care standards, leadership and the performance management of team members.

To assess, plan, implement and evaluate individualised patient care, utilising their professional judgement at all times.

Provide clinical leadership and line management to less experienced Nurses, and other members of the multidisciplinary team in the delivery of high quality care.

To contribute to the continuous improvement of community nursing within their Place, linked to the 10 year NHS plan.
