Job summary

The Senior Care-Coordinator role is an integral part of the Team Up team in Swadlincote. You will be working with a team of GP's, ACP's, ECP's and other local health professionals to deliver an outstanding service to Housebound Patients in Swadlincote.

We are a newly formed team providing wholistic comprehensive patient care. The Senior Care-Coordinator is a vital role in ensuring we all work efficiently and effectively making sure the right care is delivered on time to our patients.

Main duties of the job

The Senior Care Co-ordinator will work within the Team Up team to support the clinicians with the administrative tasks that are generated from new patient assessments and ongoing work required to manage the active workload.

The post holder will assist the ECP team with waiting list management. This would involve monitoring of clinician workload to ensure that all slots are being utilised and to monitor task boxes and caseloads for appointment booking.

The Senior Care Co-ordinator will be required to contact patients to gather information to support clinicians in preparation for a visit and for assisting the team to make decisions regarding waiting list management.

The post holder will provide day-day management for the team of DCHS Care Co-ordinators including leading the team, managing absence, conducting appraisals, reviewing work performance and ensuring wellbeing.

Co-ordinate weekly CST for Team Up and Monthly Complex MDT for wider Practices and Healthcare teams.

Use local data sources including Public Health Data, PCN data, Practice data and data from other Place partners to recruit patients onto the ECP service.
