Job summary
We are seeking to recruit an enthusiastic and motivated Healthcare Assistant to join our Derby City Rapid Response D2A team. Our office base is at Florence Nightingale Community Hospital.
This is a 37.5 hour week post requiring you to travel across Derby City, as part of the D2A team, providing holistic care to patients in their own homes.It is essential that you have experience working in either a health or social care setting.A good standard of general education including English Language is essential, along with good communication skills.
As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way.

Main duties of the job
This post will involve working within the Rapid Response D2A team visiting patients within their own homes.
We also have patients at Perth House - a shared Social Care and Health residential home, supporting patients stepping 'down' from hospital with a period of further assessment and also, stepping 'up' from the community, in order to prevent unnecessary hospital admission.
To assess, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
To provide the delivery of high quality care and actively support the D2A nursing team in the management of the health care environment.
To implement the relevant service.
To work in close collaboration with the DAU, UCR, and Integrated Community Teams.

To participate, as an individual and as team member, in continuous learning and reflective practice in order to maintain own knowledge and skills and share good practice with colleagues to continually improve services.
To actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.
