Due to expansion and promotions within our service we now have a vacancy for a Heart Failure Specialist Practitioner

Our heart failure service is looking for a highly motivated practitioner (nurse or AHP) to join our very enthusiastic and busy team; either Band 7 (if all criteria met) or Band 6 (as a training post).

We are a proactive, integrated team that works across Acute and Community settings.

We also have an integrated, tele-health team, using technology (telehealth), to support patients and have a dedicated Ambulatory heart Failure unit delivering parenteral diuretics and Iron therapy for heart failure patients.

Main duties of the job

This post is multi-faceted and involves working predominately within the Royal Derby Hospital. However, we also involve cover the community (home visits/clinics) patients at times.

Within the acute trust, you will be reviewing patients who have been admitted to hospital with heart failure symptoms. Working within the virtual ward, ambulatory service and in-reaching to hospital wards

We aim to prevent avoidable admissions and support patients admitted with heart failure and support early discharges.

You will also be working within the Ambulatory HF unit is where we deliver our IV therapies (iron and diuretics).

In the community, you will manage a caseload of complex heart failure patients - this includes managing acute decompensations and optimising therapies.

We work closely with the cardiac physiologists for device management and have weekly MDT's with the heart failure cardiologists at Royal Derby Hospital.

We also link in with other specialities for MDT's so apart from working closely with the cardiologists and physiologists we work closely with the respiratory, renal, palliative care and endocrine teams to support patient care.

  • Professional qualification and on associated professional register
  • Teaching and assessing in clinical practice qualification (eg ENB 998, C&G 7307 or equivalent)

Desirable

  • NMP
  • HF module level 6/7
  • Clinical assessment skills

Experience

Essential

  • Community experience or acute hospital experience caring for heart failure patients
  • Experience adjusting/titrating therapies/reviewing HF patients
  • Evidence of managing own caseload and working within team

Desirable

  • Community AND acute trust experience with heart failure patients
  • Recent leadership role
