Job summary

We are seeking Advanced Therapy Support Workers with a patient-centred approach and a passion for caring in the community

We are developing a new service providing assistance to people who have fallen but are unable to get up without help. These patients have historically waited a long time for assistance as they are not a priority for the ambulance service. We require advanced therapy assistants to support with this

Our teams will be provided with the right training and equipment to support and provide ongoing care as needed

Alongside the falls recovery service you will also work across our core and rapid response teams. Our integrated, multi-disciplinary teams provide timely intervention that is individualised, goal-focussed supporting patients in their own homes and local care homes

We aim to prevent unnecessary admissions and facilitate timely discharges from acute and community beds and allowing people to remain in their own homes for as long as they are able. Whilst also promoting patient independence with an holistic approach, providing therapy services in their own homes with equipment to support with this

You will be well supported in the role by a skilled, multi-disciplinary team, including Physiotherapists, Occupational Therapists, Nurses, Support Workers and Care Co-ordinators

Interviews will be held Monday 8th January at Whitworth Hospital, Matlock

Main duties of the job

To perform and assist in the provision of holistic, high quality care, which may include assisting patients from the floor following a fall.

To work within an appropriate supervisory and monitoring framework, following assessment proformas, agreed multi-disciplinary treatment plans and by providing and managing ongoing treatment, teaching and instruction to a referred caseload of patients.

The post holder will be expected to implement, modify and evaluate these programmes of treatment without direct supervision.

This will include rapid intervention, triage, initial assessment and reviews of patients, creating specific patient led care plans, meeting goals and liaison with other specialist services within the Trust.

The post holder will organise and prioritise their own day- to-day work tasks and activities, be able to work independently and adhere to Trust Lone Working policies, as well as delegate tasks to band 3 colleagues and support administrative tasks for the day-to-day running of the services.

As part of the role there will be a need for extensive travel and transportation of equipment around the Derbyshire Dales

Qualification & Training

Essential

  • Willingness to undertake further recognised training programmes applicable to the role.
  • Good standard of general education, including English Language and Maths GCSE or equivalent.
  • NVQ Level 4 in Health and Social Care, Foundation degree

Desirable

  • Assistant Practitioner Qualification or equivalent

Experience & Knowledge

Essential

  • MDT or integrated team working experience.
  • Experience of working with patients who present with complex multi-pathologies.
  • Experience in using recognised falls prevention techniques, including outcome measures.
  • Understanding of factors that influence health and well-being.

Desirable

  • Experience of working within a Community based team.
