Calling all Band 5 Registered Nurses! We have vacancies for Community Nurses.
We have roles available for you within our Derbyshire, Amber Valley Community Teams. Full and part-time hours available.

We have bases in Belper, Ripley, Alfreton and Heanor.

Main duties of the job
Provide high quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurses in the management of the health care environment.
Implement/refer on to the relevant services as appropriate.
Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
