We have a fabulous opportunity for a Pulmonary Rehabilitation Therapy Support Worker and the team are looking for a motivated individual with exercise and rehabilitation experience to join our evolving team.
Our team purpose is to improve the lives of our patients, who have chronic lung disease, by instilling a greater sense of health, well-being, and independence through a combined effort of exercise, education and support.
We deliver a leading-edge Pulmonary Rehabilitation Service.
You will be based at Walton hospital and will be part of the wider Community Respiratory Service and work across different community hospitals and community settings within the North Derbyshire area, including the High Peak.
Working in a healthcare support role, it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way. Main duties of the job
The post holder will:
- support the delivery of pulmonary rehabilitation exercise and education programmes in North Derbyshire; organised by the Specialist Respiratory Physiotherapist to encourage patients to gain their optimum level of function and independence.
- facilitate the efficient utilisation of resources and to contribute to ensuring that specific contractual requirements are achieved in accordance with service specifications and NHS National Standards.
- as a member of the DCHS North Derbyshire Community Respiratory Service to contribute to the strategic and business development of the wider service.

The service provides evidence-based, streamlined, structured care within the community, and is responsive to patient need. The service works as part of a wider respiratory pathway for acute and chronic respiratory disease management and in line with the strategic objectives set out in the NHS Long Term Plan.
The Community Respiratory Service focuses on individual care planning and enabling patients to effectively manage their own care and therefore reduce the number of emergency admissions.

Patient Care
- Work under the guidance and direction of the Physiotherapist, assisting them to carry out treatments or designated treatment programmes. This work may be supervised or unsupervised.
- During pulmonary rehabilitation groups, lead exercises for warm up and cool downs, supervise exercise programmes and assist in the progression of exercise. Provide education presentations for respiratory patients on the programme.
- Perform outcome measures as required at baseline and final testing and assist the physiotherapist in scoring outcome measures.
- Assess and provide training in the use of equipment and to order equipment from other agencies for patient use.
- Complete home exercise programmes as required as prescribed by the physiotherapists
- Communicate effectively and sensitively and use language appropriate to patient and carer/relatives condition and understanding.
- Effectively use all forms/methods of communication and be aware of and manage barriers to communication.
- Effectively recognise and manage challenging behaviours in patients, carers and or relatives.
- Provide information to the team, patient, carer and or relatives.
- Respect the dignity of individuals, obtaining consent and involving them, their carers and or families and relatives in shared decision making.
- Effectively and efficiently undertake all aspects of clinical duties and procedures required and identified in an individuals treatment plan, recognising own competencies and the scope of own role.
- Assist in handling patients, manually, using the hoist and other equipment including wheelchairs as needed.
- Report clearly and discuss patients treatments with MDT members and other relevant agencies to maximise rehab treatments/clinical outcomes

- Adhere to Derbyshire Community Health Services policies and procedures.
- Contribute to clinical audit and service evaluation and promotion

Financial Resources/Management
- Provide non-clinical support to the service by assisting in clerical/admin duties as required.
- Care for the equipment used in the delivery of Pulmonary Rehabilitation and other areas of work, using infection control measures as policy demands
- Arrange for equipment to be maintained on an annual basis, to ensure patient safety
Information Resources/Information Systems
- Keep accurate and up to date records of patient treatments and other interventions, compliant with legal guidelines and keep statistics and input data as required
Autonomy/Scope within Role
- Assist in the delivery of Pulmonary Rehabilitation exercise and education programmes under supervision of the specialist respiratory physiotherapist in Community venues across North Derbyshire.
- Work within clearly defined occupational policies, protocols, procedures and relevant codes of conduct, performing duties identified within individual treatment plans.
- Implement and evaluate programmes of care without direct supervision, but will be expected to liaise closely with relevant professionally registered staff.
