Job summary

We are seeking to recruit an enthusiastic and motivated Healthcare Assistant to join our Care Team based at Ilkeston Community Hospital providing end of life care for patients in the Erewash area.

Erewash Care Team are friendly and supportive taking great pride in the service that they delver which supports and cares for patients in the final weeks of their life.

You will be required to travel across Erewash, as part of a wider integrated team providing holistic care to patients in their own homes, sometimes in conjunction with the District NursesIt is desirable that you will have experience working in either a health or social care setting.A good standard of English is required, along with good communication skills.

As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way.

Main duties of the job

This post will involve working within a community nursing team visiting patients within their own homes.

The post holder will:

Assess, implement and evaluate individualised patient care at the end of life to the highest of standards whilst maintaining the patient's privacy and dignity.

Provide the delivery of high-quality care, compassionate care involving family and carers where appropriate

Work in close collaboration with the Integrated Community Teams.

About us

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

To provide holistic care to patients in their own homes or alternative health care setting, under the supervision of the appropriate registered professional.

To participate, as an individual and as team member, in continuous learning and reflective practice in order to maintain own knowledge and skills and share good practice with colleagues to continually improve services.

To actively contribute to protecting people at risk by assessing and monitoring the care environment and individuals and by following and acting on and within Safeguarding policies and procedures.
