Job summary
We are seeking a Band 4 Advanced Therapy Support Worker with a patient-centred approach and a passion for assessment and rehabilitation to work throughout Derby city.
This would be a great development opportunity for an experienced Band 3 Therapy Support Worker, as training and support will be provided.
Our integrated, multi-disciplinary team provides goal-focussed rehabilitation to support patients in their own homes and local care homes.
The Rapid Response service is provided 8am-6.30 pm. every day.
The service aims are to prevent unnecessary admissions, pathway 1, pathway 2a and discharge 2 assess service from hospital.
You will have the opportunity to develop your skills in assessment and treatment in a community setting alongside developing additional generic skills to provide a comprehensive assessment to a variety of complex patients.
You will be well supported in the role by a skilled, multi-disciplinary team, including Physiotherapists, Occupational Therapists, Nurses, Support Workers and Care Co-ordinators.
As part of the role there will be a need for extensive travel and transportation of equipment and between patients around Derby city.
Main duties of the job
To perform and assist in the provision of holistic, high quality, evidence-based health care. to a defined local population.
To work within an appropriate supervisory and monitoring framework, to agreed multi-disciplinary treatment plans by providing and managing ongoing treatment, teaching and instruction to a referred caseload of patients.
The post holder will be expected to implement, modify and evaluate these programmes of treatment without direct supervision.
This will include initial assessment and reviews of patients, creating specific patient led care plans and meet therapy goals and liaison with other specialist services within the Trust.
The post holder will organise and prioritise their own day- to-day work tasks and activities, be able to work independently and adhere to Trust Lone Working policies, as well as delegate tasks to band 3 colleagues and support administrative tasks for the day-to-day running of the services.

- Good standard of general education, including English Language and Maths GCSE or equivalent.
