High Peak & Buxton PCN (Primary Care Network) is seeking an Advanced Practitioner to be part of our multi-agency team of professionals working across the High Peak.
We are looking for an Advanced Clinical Practitioner/Advanced Nurse Practitioner with a strong interest in working with frailty and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred, and effective care within a neighbourhood team environment.
The service, within which the post holder would work, supports patients with multifaceted health problems living in their own homes as well as in care homes.
The service liaises with care home staff, carers, families, and colleagues to maintain and maximise individualised patient-centred care.
Main duties of the job
The successful post holder will work autonomously within their professional boundaries to:
- Undertake comprehensive health assessments, initial history taking, clinical assessment, physical examination, diagnosis, treatment, and evaluation of their care
- Use advanced clinical decision making and assessment skills, coordinate care planning and initiate onward referrals as appropriate
- Demonstrate sound safe clinical decision-making.
The role will support the newly formed Acute Home Visiting Service (AHVS), working with the GPs to triage referrals, undertake home visits and provide appropriate care for housebound patients.
The role requires a high degree of autonomy, and the successful candidate will have excellent interpersonal and communication skills.
The candidate will be a confident and adaptable clinician who can work at an advanced clinical level using independent prescribing (V300) & clinical guidance documents.
The successful candidate will contribute to the development of relevant policies and protocols whilst delivering service improvement for the service.
This role will involve travelling to housebound patients across the High Peak as and when required. There will be a requirement to work evenings and weekends as the service develops.
A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice and who can demonstrate contemporary experience working at an advanced level within a relevant, clinical environment.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
This post will be hosted by DCHS on behalf of the High Peak & Buxton PCN (Primary Care Newtwork). The PCN consists of 8 GP practices (up to 65,00 patients) across the area which currently operates as one PLACE.
