Band 7Project Lead - Implementing Personalisation and Quality Conversations in an Acute Hospital Setting

This project leadrole offers a fantastic opportunity for an interested post holder to promote a Personalised Care and Quality Conversations approach, in an acute hospital setting.

This exciting project is a key part of local work to address health inequalities locally, and improve the health of Derbyshire residents.

The postholder will focus on workforce development, in order to embed a Personalised Care and Quality Conversations approach, within targeted services/pathways in Chesterfield Royal Hospital and Royal Primary Care.

You will become a part of the Derbyshire Psychological Insights team (a team with a range of psychological and behavioural expertise), which will enable you to draw on peer support, and a network of colleagues in similar roles.

Main duties of the job

The post holder will:

  • Scope current practices in terms of health prevention and promotion in targeted clinical areas
  • Share, adopt and spread personalised approach to healthcare practice
  • Introduce personalisation interventions with interested teams
  • Support staff to have more shared decision-making conversations that consider the wider determinants of health
  • Embed learning from the Quality Conversations training to support sustained staff and patient behaviour change
  • Identify organisational needs and create a delivery plan to transfer the knowledge to teams, including measuring the impact for patients and staff
  • Ensure sustainability by creating a personalisation legacy, share new pathways and learning across the transformation team and wider clinical teams

This post is being offered for 12-18 months and is open to a range of staff, who meet the specified essential criteria.

Education, Qualifications and Training

Essential

  • Masters degree level qualification or equivalent experience
  • Project Management qualification or relevant experience
  • Quality Improvement/Behaviour Change Training

Desirable

  • Leadership Qualification/training

Experience and Knowledge Required

Essential

  • Comprehensive knowledge of the Comprehensive Model of Universal Personalised Care and the Quality Conversations Programme
  • Knowledge and experience of addressing health inequalities
  • Demonstrable experience of driving service improvements and innovation.
  • Comprehensive knowledge and experience of Microsoft Office including Microsoft Excel, Teams, SharePoint and Powerpoint
  • Work within a Programme Office in a complex transformational environment

  • Experience of Personalised Care, Behaviour Change or Communication Skills projects/programmes within the NHS or a large public sector organisation
