Job summary

We are looking for an experienced GP/Consultant Geriatrician to lead a new multi-disciplinary team delivering home visits to the housebound patients within the Swadlincote Primary Care Network.

You will have an interest in house bound patients, care of the elderly and delivering care in the most appropriate and efficient way possible.

Team up is a Derbyshire initiative bringing together local Health, Social Care and Voluntary Sector provision to deliver the best care for our patients.

We will consider full-time or part-time working for this role.

Main duties of the job

If you are the kind of person who is interested in developing and growing new ways of delivering Primary care; leading a team of high performing senior clinical staff (GPs, ACPs, ANPs, Matrons); working in a collaborative way; supporting the professional development of others and delivering care in a holistic way then this is the job for you.

You will be employed by Derbyshire Community Health Service NHS Foundation Trust delivering care for the patients registered with the 5 practices within Swadlincote Primary Care Network.

You will work closely with the GP's and other staff within the PCN, the Home Visiting Service Team and the wider DCHS Community Teams. The key responsibility will be triaging and managing referrals to the HVS and allocating them to the most appropriate clinician in the team.

You will also provide support, advice and guidance to the team. Leading the development of the team and the HVS alongside the PCN is also a significant part of the role.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

  • Current registration with the GMC

Desirable

  • Royal College membership
  • Mentorship and Supervision Qualification or equivalent experience (eg GP Trainer, DMP)
  • Mentorship and Supervision Qualification or equivalent experience (eg GP Trainer, DMP)

Experience and Knowledge Required

Essential

  • A good knowledge of and interest in community care
  • Experience of leading a team of senior clinicians
  • Experience of working with multidisciplinary teams
  • Experience of supporting Urgent Treatment Centres or an aspect of emergency care or a willingness to develop skills in this area with Trust support (If that is your desired area of alignment)
