DCHS Children's Services are have an exciting opportunity to offer an experienced SCPHN Health Visitor to be a part of the Intensive Health Visiting Team within 0-19 Children's Services based in North East Derbyshire

As the new postholder you will deliver an intensive home visiting service to vulnerable women and their families. You will be required to develop therapeutic relationships with clients and work intensively within complex family situations from pregnancy until the child reaches 2.5 years of age, providing family focused evidence-based care.

You will be invested in our business, creating a culture of continuous improvement, where family welfare is at the heart of all you do.

You will provide services in collaboration with partner agencies, statutory organisations and the voluntary sector and identify and act on the public health needs of the 0-2.5 years population, working together to improve service provision and outcomes for this population group and their families, both within your locality and across the service.

Main duties of the job

Main duties of the job

You will be responsible for delivering the Intensive Health Visiting Service within Derbyshire to vulnerable, hard to reach young women and their families from the antenatal period through to 2.5 years.

You will have responsibility for a maximum caseload of 60 clients/children.

You will play an integral role in developing the 0-19 Children's Service's Team in their delivery of the commissioned service specification.

You will be part of the multi-disciplinary team including the wider local children's workforce, working within a public health framework to meet the prioritised health needs of children, young people and families.

You will deliver the Healthy Child Programme at universal partnership level and above, following the enhanced service pathway, along with progressive levels of service according to identified need.

You will possess excellent communication skills to effectively engage with your client group, peers and partner agencies in all elements of your role, leading by example and promoting a supportive and caring culture within your team.

You must be a competent and experienced safeguarding practitioner.
