Job summary
We have an exciting opportunity for a registered nurse to join the North Urgent Community Response Team on a 12 month secondment / fixed term contract, to support with urgent community 2 hour response for patients in their own home, with skills in palliative / end of life care, wound care and catheter care and a strong desire to further develop your community nursing skills and experience.
You will be required to work over a 7 day working week including weekends and will be hard working, motivated, with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic assessment, to demonstrate that you use your initiative and have a keen interest in working with this group of patients. You must hold a full and valid driving licence and have access to a vehicle.
You will support staff working at a band 3 and 4 level, along with students and apprentices.
Main duties of the job
As part of the Urgent Community Response (UCR) Team you will be required:o To work within the Urgent Community Response Team to prevent unnecessaryadmission to hospital.o To assess and prescribe care pathways for patients in a crisis situation withlong term conditions, the frail & elderly, palliative and end of life care and/orrehabilitation needs, to achieve quality of life and independence wherepossible, with support from Senior colleagues within the wider Urgent Community Response Team.o To act as the patients advocate, facilitating choice and patient empowerment.o To provide evidence based clinical/therapeutic interventions based on "bestpractice".o To work within the Urgent Community Response Team to support earlydischarge from hospital.o To work with all health care professionals, and statutory/non statutory agenciesto provide a seamless, integrated service to our service users.o To triage referrals on a rotational basis for the Urgent Community Response Team.
