Job summary

An exciting opportunity for a Community Assistant Practitioner or Nursing Associate, has become available in the Downham Market and Swaffham Primary Care Network Team. This position is based in Downham Medical Practice to Cover the Fens and Brecks area. The post can be offered as part time or full time.

Main duties of the job

As part of the Fens and Brecks Community Nursing Team you will provide interventions for patients in their own homes, care homes, and other community settings. You will be able to work autonomously and in an integrated way with our partners in Health & Social Care. The ability to work as part of a team and to have an adaptable approach to work is essential.

Working within the team will include a variety of patients, conditions and tasks including:

  • Palliative Care
  • Wound Care
  • Long Term Conditions - i.e. Insulin administration
  • Catheter and Bowel care
  • Venepuncture

We offer comprehensive training, supervision and you will be supported by a a friendly and professional team, including an experienced Clinical Lead Nurse and Clinical Operations Manager.
