Clinical Lead Nurse - Triage and Allocation (North Place)

Job summary

We are looking for a dynamic, enthusiastic and dedicated Clinical Lead Nurse to join our triage and allocation team.

This role will provide the clinical oversight and leadership for the referral to allocation process and be instrumental in co-ordinating service and quality improvements in North Place.

Due to the urgent nature of this role, it is essential that applicants have experience in triage.

Main duties of the job

To provide professional and clinical leadership within the locality.

To support staff to achieve and ensure competencies are maintained within the team.

To ensure that Trust annual priorities and strategic objectives are embedded within the workforce by ensuring that all annual appraisals are complete within the team.

To act as a Hub Co-ordinator to support the management of patients with healthcare needs who require ongoing monitoring and clinical intervention alongside providing advice to front line health care professionals as required.

To provide professional leadership to own professional group.

To provide clinical leadership within the locality.

To work with, and deputise for, the Clinical Operational Manager as required.

To oversee and monitor the referral, triage and allocation process to ensure to ensure patients' needs are met in a timely and effective manner.

To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

To support the triage team by having 1:1's to review caseload.

To oversee that clinical supervision is occurring within the team andaccurately recorded.

Clinical

The post holder will need:

1. To be professionally and legally responsible and accountable for all aspects of the post holders work, including the management of people under the post holders professional care.

2. Assessment, planning, implementation and evaluation of all aspects of patient care, and development treatment/care plans that may need to be delivered from a range of options.

3. Review patients holistic needs, communicating highly complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan using advanced skills in both verbal/non-verbal communications.

4. Be aware of Assistive Technology and utilise it in patient care where appropriate.

5. Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.

6. Be required to use tact and persuasive skills in order to gain the patients co-operation in their treatment/care/management plan.

7. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.

9. Plan and organise specialist services within the integrated team.
