Job summary

This is a part-time position working as a Band 6 Children's Community Nurse in an established and supportive team. An opportunity to gain clinical and leadership skills, you will work as an autonomous practitioner leading within the Children's Community Nursing Team to provide clinical intervention and support to children, young people and their families who have complex or acute health needs.

Main duties of the job

Managing a clinical caseload and provide day to day management and support to junior staff

Providing direction and leadership to junior staff including management supervision and clinical oversight.

Assisting junior staff to prioritise, implement and evaluate patient care.

Completing holistic assessments of CYP, identifying their nursing needs.

Developing, implementing and evaluating appropriate care plans / comprehensive packages of care.

Providing quality care to children in partnership with families and carers.

Delivering direct patient care to meet the physical and emotional needs of the child / young person, taking account of specialised (enteral) feeding, respiratory (tracheostomy, Ventilator), manual handling and medication regimes (sub-cutaneous infusions, interventions).

Undertaking risk assessments and completing Datix reporting.

Communicating effectively with child, family and wider multi-agency staff regarding patient wellbeing needs to ensure continuity of care.

Attending/ Chairing team meetings, Trust meetings, training sessions and courses as required.

Acting as a specialist support, educator, and resource to all other relevant professionals.

Regularly assess the teaching of specific nursing techniques, including invasive procedures to identified staff / parents / carers/ student nurses / apprentices.

Proactively contributing to the EHCP process.
