Job responsibilities
Clinical
Manage a clinical caseload and provide day to day management and support to junior staff who are associated with a caseload
Provide direction and leadership to junior staff including but not limited to management supervision and clinical oversight.
Assist junior staff to prioritise, implement and evaluate patient care in accordance with Trust policies, procedures protocols and guidelines, while adhering to these yourself.
To complete a holistic assessment of the CYP identifying their nursing needs.
To develop implement and evaluate appropriate care plans and comprehensive packages of care.
To provide quality care to children in partnership with families and carers as commissioned and described within the service specifications.
To deliver direct patient care to meet the physical and emotional needs of the child / young person, taking account of specialised (enteral) feeding, respiratory (tracheostomy, Ventilator), manual handling and medication regimes (sub-cutaneous infusions, interventions).
To take all possible steps to ensure the safety and welfare of children and staff.
To carry out all relevant aspects of care adhering to NCH&C Trust Policy / procedure and Nursing and Midwifery Council guidelines
To undertake risk assessments and complete Datix reporting as required
To communicate effectively with child, parents / carers and the wider multi-agency staff regarding the childrens wellbeing needs to ensure continuity of care. Strengthen communication between all parties by recognising barriers to effective communication and acting sensitively and compassionately to overcome these.
To offer advice, guidance and reassurance to families as needed.
To attend, contribute and chair team meetings, attend Trust meetings, training sessions and courses as required.
To act as a specialist support, educator, and resource to all other relevant professionals.
To provide, implement and regularly assess the teaching of specific nursing techniques, including invasive procedures to identified staff / parents / carers/ student nurses / apprentices in a variety of settings.
To work proactively to contribute to the EHCP process.
To actively engage in the development of protocols and guidelines and care pathways for the management of children with complex and / or acute health needs.
Promote and develop the use of current evidence to inform the delivery; and promote the development of clinically effective and safe care.
Provide initial triage and assessment for new referrals to the service, delegate to appropriate others as required.
Lead and develop effective discharge from hospital to home or respite setting, carrying out risk assessments in line with continuous care assessment to ensure seamless transition to home or into a respite setting.
Plan and allocate work in order of priority and delegate to colleagues and direct reports, to skills and competencies.
Professional
To support the clinical leads, Childrens matron and lead paediatric nurse for complex health and disability to meet the changing needs of children/young people with complex or acute health care needs.
To work with clinical leads and the childrens matron to develop evidence-based practice by utilising research, and auditing clinical effectiveness.
Ensure that all practice is in accordance with the most recent NMC code: standards of conduct, performance, and ethics: standards for medicines, management, management, and standards of record keeping.
To carry out all relevant aspects of care adhering to the NCH&C Trust policy/procedure and NMC guidelines and standards including where appropriate the NMC standards or Proficiency for Nurse and Midwife Prescribers
Maintain awareness of clinical and professional developments in using and participate in nursing research where it is appropriate to do so, to enhance patient care.
To actively participate in the Trust, management, clinical supervision and safeguarding supervision and where appropriate safeguarding supervisors supervision
To initiate and contribute to the development and introduction of new ideas and ways of working and act as a source of advice to the benefit of the child and young person and the development of the service.
At all times ensure that own actions support and promote equality, diversity, and the rights of the public and colleagues within the health care environment.
Provide support, guidance and supervision to other staff as required.
Management and Leadership
Manage, deploy, supervise, and delegate work to junior staff and perform their appraisals.
Deputise for the clinical lead in their absence.
Deputise where appropriate in meetings for other team members or the clinical lead.
Supervise and Assess students.
Conduct new staff induction and training.
Work in collaboration with members of the multi-disciplinary team, specialist staff and tertiary centres
Identify issues relevant for audit and participate as required.
Ensure all equipment in use is monitored and checked as per protocols.
Understand and contribute to using the available resources effectively.
Identify hazards and risks and deal accordingly to actively promote the health and safety of patients, the public and staff in the health care environment.
Financial responsibilities
Ensure that NHS resources including property, equipment, time are used appropriately.
Take personal responsibility for effective management of resources and safe use of equipment.
In conjunction with the Childrens Matron, Lead Paediatric Nurse, Service, and clinical leads, assist in the understanding and delivery of approved Cost Improvement Plans.
Information Management
Maintains accurate records electronically in a professional manner and in accordance with both local Trust policy and NMC guidance.
Be aware of relevant trust policies.
Is aware of the Data Protection Act and ensure that patient confidentiality is maintained, and patient information is protected.
Complete all necessary documentation and ensure confidentiality.
To write comprehensive and concise reports where necessary.
Education and professional development
Comply with the NMC code of professional conduct in relation to maintaining a portfolio and undertaking 3 yearly revalidations.
Be responsible for your own development, applying to undertake relevant training when available.
Actively seek educational opportunities to ensure best practice is delivered to patients.
Share knowledge gained with colleagues.
Operational Delivery
Ensure the maintenance of adequate staffing levels; utilising E-roster and SystmOne Appointment Rota software as appropriate. Risk report and escalate to the clinical lead, modern matron or Lead Paediatric Nurse when required resources cannot be achieved. Ensure the roster is made with adequate consultation with staff and timely (according to Trust policy).
Respond to staff changes and recruitment needs facilitating continuity of the service.
Support the Clinical Leads in planning service provision, including resource management.
Leadership and management of the Nursing Team, including Appraisals, mandatory training, and the development of the team in response to changing needs of service delivery.
Risk and Governance Management
Highlight risks to clinical leads to place on the team risk register.
Contribute to the monthly review of the team risk register.
Service Development
Comment on and contribute to relevant professional policies and procedure.
Attend policy working groups to ensure childrens services are considered within Trust policies.
Assist in or develop innovations or new practices as the service require.
Communication and Relationship Building
Communicate effectively complex and sensitive information with patients, their families and health professionals both within and external to the organisation, adjusting the level of communication to suit the situation. Recognise barriers to communication and work collaboratively to overcome these.
