Clinical Lead - Triage and Unplanned Care Nurse

Job summary

This is an exciting opportunity to assist in leading our ever changing and improving Unplanned Care Team, Triage team and Peripatetic Insulin Team in West Norfolk as a Clinical Lead based within West Triage and Allocation Hub in St James Clinics.

This role will predominantly involve face to face nursing care and assessment within a patient's usual place of residence, with an element of Triage for the Community Nursing Teams and Unplanned Care team on a rota basis as well as other supportive activities as part of the Multidisciplinary Team (MDT). Supporting Management in clinical supervisions, team development and appraisal, assisting with team dynamics changes and development.

You will be expected to support conversations with the wider Urgent Community Response (UCR) Multidisciplinary Team (MDT) on a regular basis enabling safe, appropriate and patient centred care to be delivered to this complex patient group.

This is a 7 day a week service and some weekend working will be expected. Hours of service delivery are, 8am-8pm, 365 days per year.

Main duties of the job

To undertake the role of management and leadership of the Triage, Unplanned Care Response and Peripatetic Insulin Teams ensuring the development and delivery of effective services that will improve the outcomes for patients.

To lead the triage team within the locality hub in the effective and timely triage of incoming referrals assessing the patient's needs, determining the priority of care and processing the referrals accordingly.

To undertake clinical duties to meet the needs of the service and ensure effective provision of safeguarding functions.

To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care. To facilitate/support Newly Qualified Nurses in their Preceptorship year.
