Senior Out of Hours Nurse

Job summary

As an exciting time to join the Urgent Community Response Out of Hours Team, we are looking for an enthusiastic and dedicated Senior Nurse to join our team.

You will be a dynamic, motivated team player that enjoys working overnight and the autonomy of community working.

Experience of dealing with palliative care, catheter care and triage is preferred but full training will be given.

Main duties of the job

To lead in the delivery of high standards of health care to patients within theirown place of residence by assessing, planning, implementing, and evaluating carein order to prevent avoidable admission to hospital and facilitating early supporteddischarge.

You will be expected to lead conversations with the wider Urgent Community Response(UCR) Multidisciplinary Team (MDT) on a regular basis enabling safe, appropriate andpatient centred care to be delivered to this complex patient group.

This role will predominantly involve face to face nursing care and assessment within apatient's usual place of residence, with an element of Triage and allocation for the UrgentCommunity Response Team on a rota basis plus other supportive activities as part of theMultidisciplinary Team.

This is a 7 day a week service and some weekend working will be expected. Hours ofservice delivery are, 8pm-8am 365 days per year.
