Job summary

The West Home First Hub team are pleased to advertise a band 6 Clinical Co-ordinator opportunity within our friendly and supportive team based at St James Clinic, Kings Lynn.

The aim of our service is to provide evidence based triage and case management of Discharge to Assess and Admission Avoidance referrals. You will be working closely within an integrated Health and Social Care team to support patients with complex health and social needs.

We are looking for a dynamic, forward-thinking Health or Social care professional with effective communication, leadership, and organisational skills. The skill set for this role may not be part of your current portfolio, however we can offer support and leadership, and are keen to discuss the role with interested candidates.

Main duties of the job

To contribute to the review and assessment of health and social care needs for the Discharge to Assess pathways providing safe discharge plans and support flow from the acute hospital.

To triage, assess and co-ordinate responses to Admission Avoidance referrals.

To undertake Trusted Assessments for patients within the acute Trust requiring pathway 2 discharges
