Job summary
We are seeking a Band 6 Physiotherapist to join the Norwich Community Therapy Team, based at Norwich Community Hospital.
The community provides an excellent opportunity to assess and treat patients with varied and complex conditions, from the age of 18 years old. This covers all the core areas of Physiotherapy. The role includes community home visits, Physiotherapy Clinics and Physiotherapy telephone triage.
You will be working within a supportive team of Physiotherapists, Occupational Therapists and Assistant staff. Regular support is available from highly experienced Therapists and Nursing colleagues. Therapists work collaboratively with other services within NCH&C, Primary Care services, Social Services and voluntary organisations, to provide holistic healthcare.
Monthly Physiotherapy peer support meetings provide opportunities for regular reflective practice and teaching. Service development is actively encouraged. Staff wellbeing is integral, as demonstrated by strong staff retention in the team, and 'Outstanding' CQC rating.

Main duties of the job
-To assess and provide therapeutic input, rehabilitation and guidance to adult patients over the age of 18 years old
-Working with a broad range of specialties including: Neurology; Frailty, Orthopaedic rehabilitation; New and chronic musculoskeletal conditions; Palliative care; Chronic Pain; Rheumatology.
-To support with weekly Physiotherapy Triage (telephone)
-To lead regular Physiotherapy Outpatient Clinics at the community hospital
-To determine clinical diagnosis and treatments, to maximise quality of life and independence where possible
-To provide timely assessment and escalation of patients with rapidly deteriorating conditions. Flexibility of caseload is needed so support the urgent care response (UCR) service occasionally.
-To provide ongoing evaluation of patient progression and rehabilitation potential
-Working autonomously in the community, under the support of senior Therapists
-To obtain relevant information from a variety of sources. This may include medical records and handover from other health and social care professionals
-Completion of environmental home assessments and equipment provision
-Completion of onward referrals to appropriate services
-To work with other health care professionals, statutory and non-statutory agencies to provide a seamless integrated service to our service users
-To provide advice, support and supervision to junior staff, therapy assistants and students
-To complete regular caseload management

The team is led by an Occupational Therapy and Physiotherapy Manager. Management support is available daily.
