An exciting opportunity has arisen in Norwich for an experienced clinician to join Norwich Place as a Therapy Clinical Lead for Home First Hub.
The successful candidate will provide clinical leadership and line management to Therapy staff working in the Home First Hub, UCR, R&R beds. NEAT and Alder ward. The successful candidate will support staff to achieve and ensure competencies are maintained in addition to ensuring that the Trust annual priorities and quality goals are embedded within the workforce.
The successful candidate will be responsible for the professional leadership of a defined team, demonstrating a supportive and flexible leadership approach and a proactive, responsible attitude to promote confidence in the team and the service.
Main duties of the job
The main duties and responsibilities of a Therapy clinical lead include providing clear operational leadership and line management, developing strong, transparent working relationships with key stake holders including Norwich PCN Neighbourhoods, the local acute hospital, Social Services, Norfolk First Support and the voluntary sector.
You will use your clinical skills and knowledge to complete the facilitation and triage of referrals for patients on discharge to assess pathways from acute and community hospitals. This will involve assessing the patient's needs, determining priority of care, and processing and allocating referrals accordingly. Effective communication skills are required to liaise with the multidisciplinary team to ensure timely responses to referrals are achieved. Telephone assessments and reviews of individuals in their own homes or care homes following discharge will be undertaken and appropriate therapy intervention required planned to meet patients needs
You will be responsible for Managing a diverse Therapy Team, providing direction, support and guidance. Implement policies for the service, proposing changes to these, and advising other external agencies about these changes or developments. To be responsible for Continued Professional Development (CPD) and Personal Development Plan (PDP) for members of the therapy team.

- Formulate and execute individualised therapy treatment plans of patients including those with complex presentation, in collaboration with the integrated team, other agencies and the patient, utilising a range of treatment skills.
- To provide assessment of functional ability and safety of patients in the community to identify care, equipment, adaptation and therapy needs.
- Assess and plan treatment including teaching patients to utilise techniques and equipment to maximise their functional ability.
- Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
- Be required to undertake risk assessments and act upon them appropriately.
- Contribute to the staff rota on a monthly basis.
- Contribute to the supervision of workload within the integrated teams.
- Actively contribute to multidisciplinary team meetings and other meetings where appropriate, and chair meeting in the absence of the Service Lead.

- First level degree or diploma in Occupational Therapy or Physiotherapy
- Current HCPC registration OT or PT
- Significant post registration experience
- Recognised management qualification or evidence of working at that level
