Job summary

An opportunity has arisen for a permanent Clinical Lead Nurse position within the Urgent Community Response Team which encompasses the Virtual Ward Norwich, Unplanned 2 hrs response and Urgent community response care (previously known as HomeWard).

Within this job you would provide expert Nursing advice and Clinical leadership to develop and supervise staff members, including Nurses, Occupational Therapists, Physiotherapists and support staff by supporting staff to achieve and ensure competencies are maintained within the Urgent Community Response team.

As well as being a professional and clinical leadership within the Urgent Community Response Team and Virtual ward Norwich and contribute in the delivery of high standards of health care to patients within their own place of residence, by assessing, planning, implementing and evaluating packages of care in order to prevent admission to hospital and facilitate early supported discharge.

When avoiding hospital admission you may be supporting the management of patients with healthcare needs who require ongoing monitoring and clinical intervention alongside providing advice to front line health care professionals as required.

The urgent community response service operates between 8:00am - 8pm, 7 days per week, 365 days a year and you will be required to work shifts between these times.

Main duties of the job

o To provide professional leadership and supervision to own professional group within the Urgent Community Response Team.o To provide clinical leadership within the Urgent Community Response Team.o To work with, and deputise for, the Clinical Operational Manager as required.o To oversee and monitor plans of care for all complex patients on the caseload toachieve quality of life and independence where possible.o To work within the Urgent Community Response Team to prevent unnecessary admission to hospital and facilitate early discharge from hospital.o To work with all health care professionals, and statutory/non statutory agencies toprovide a seamless, integrated service to our service users. To show a level of resilience by working with the team to develop competencies in all areas of practiceincluding Occupational Therapy and Physiotherapy.

Clinical

The post holder will:

1. Be professionally and legally responsible and accountable for all aspects of the postholders work, including the management of people under the post holders professionalcare.2. Assessment, planning, implementation and evaluation of all aspects of patient care,and development treatment/care plans that may need to be delivered from a range ofoptions.3. Review patients holistic needs, communicating highly complex and sensitiveinformation to patients and carers as to their assessment, diagnosis, prognosis andtreatment plan using advanced skills in both verbal/non-verbal communications.4. Be aware of Assistive Technology and utilise it in patient care where appropriate.5. Monitor equipment and supplies via the electronic ordering systems and approve andrequest new equipment as appropriate.21.
