Brand New Role in Community

At NCH&C we are creating a new team that will receive training to ensure our patients with enhanced needs have the best possible experience whilst an inpatient at one of our wards.

The team will need a new leader that has a passion for patient experience and caring for rehabilitation patients with an appreciation for the complexity of mental health conditions.

This is a new way of working, with complete flexibility around life outside the job, and so the Team Leader will need to be responsive and have a great working knowledge of rostering, all while keeping an eye on areas that would benefit most from the teams help and knowledge. This team will travel around Norfolk to cascade training so you won't be stuck in an office behind a desk all day, but out and about meeting our teams and patients.

Main duties of the job

  • To be responsible for the day-to-day operational line management of the team, leading and ensuring a adaptable and expert service is provided.
  • To implement an appropriate training programme for HCSWs to care for patients with mental health conditions requiring enhanced care and specialist interventions in line with NCH&C Dementia and Mental Health Strategy
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to patients.
  • To provide senior leadership Monday to Friday with the expectation to cover service gaps as required.

Job responsibilities

Clinical

1. To be professionally and legally accountable for all aspects of own work and to ensure that work is within professional standards and clinical guidelines.

2. Work flexibly, managing own working time effectively according to service demands.

3. In collaboration with System Operations and the Safer Staffing Escalation Group deploy team appropriately across the Trust

4. To provide clinical leadership to direct reports and wider Trust inpatient settings

5. To provide clinical leadership in creating care plans and making recommendations on nursing interventions to support patients with enhanced needs.

6. Deploy team having assessed and prioritised patients and teams with the highest clinical need.

7. To ensure that patients, families, and carers are involved in the planning of care plans.

8. Support the review of mental health and 1-2-1 care plans.

9. Link with Dementia Lead

10. To work collaboratively with other services that may be involved in the care of the patients including Social Services, NSFT and the Voluntary sector.

11. To contribute to relevant multidisciplinary meetings and patient reviews as appropriate.

12. To provide supervision, advice and support to other members of nursing staff and assistants within the team regarding management and care of people with complex nursing needs, ensuring practice is commensurate with NMC professional standards.

13. To ensure effective communication with ward leadership and clinical teams of care needs and interventions for patients with enhanced needs. This includes ensuring effective care planning, risk assessment and escalation processes in deterioration or increased risk.
