Job summary

An exciting opportunity has arisen for an enthusiastic Registered Nurse to join the Norfolk and Waveney virtual ward triage hub. The successful candidate will be supported by a friendly and welcoming team of Nurses, Nursing Associates, senior business officers and care coordination, with the opportunity to develop leadership skills through supervision and in house training.

Full or part time hours would be considered.

The Virtual ward hub operates 7 days a week to treat acutely unwell patients in their homes, including residential homes, nursing homes and supported living complexes via Advance practitioner support and technological monitoring.

Norwich and Waveney virtual ward hub is working alongside its commissioners and local service providers Network to develop community based services that support patients to remain at home and receive the services they require in an effective and co-ordinated way.

Main duties of the job

The workload requires a good range of clinical skills and nursing experience to be applied in advanced clinical triage for patients in an acute crisis where hospital admission is a possibility, however, required to be avoided.

Many of our patients will have complex multi-pathologies and so we aim to provide holistic, patient centered assessments and deliver individualised care with in their own homes to prevent direct hospital admission

As a Senior Triage Nurse you will support the Clinical Lead Managers and triage staff in aspects such as roster creation, Datix management, staff supervision, PDR's, and to develop a good working knowledge of the nursing caseload.

Norwich and Waveney Hub works as cohesive team of Urgent care responses Nurses across 5 localities you may be asked to support across teams and advanced clinical assessment.

  • To assess and prescribe care pathways for patients in a crisis situation with long term conditions, the frail & elderly, palliative and end of life care and/or rehabilitation needs, to achieve quality of life and independence where possible.
  • To work within the integrated team to support early discharge from hospital

Clinical

The post holder will:

  • Assess, plan, implement and evaluate all aspects of patient care, and develop treatment/care pathways that may need to be delivered from a range of options remotely
  • Following a holistic assessment, devise an individualised care pathway for each patient, modifying it as required.
  • Be aware of Assistive Technology and utilise where required.
  • Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
  • Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
  • Be required to use tact and persuasive skills in order to gain the patients co-operation in their treatment/care/management plan.
  • Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
