An exciting opportunity has arisen for a band 5 Community Occupational Therapist to join our supportive Therapy team in Norwich!
Based at Norwich Community hospital, the successful applicant will visit patients in their own homes, including housing with care and care homes. They will work alongside their Physiotherapy and assistant colleagues to provide treatment plans to increase the independence of those living at home with both physical and cognitive difficulties above the age of 18.
Main duties of the job
The successful applicant will complete a holistic assessment of the individual, prepare and carry out a treatment plan, and collaborate with other Professionals including Physiotherapists, Nursing staff, Social workers, unplanned services, and many more.
Community working requires a car driver with access to a vehicle to travel to patient's homes. A laptop will be provided so that documentation can be completed at the time of the visit.
About us
The successful applicant will be provided with a comprehensive induction plan, and will ultimately work independently. Supervision is provided with a band 6 Occupational Therapist once a month and we have both Therapy team meetings and also Occupational Therapy peer supervision sessions.
The Clinical Leads are proactive in providing training opportunities to enhance skills and abilities in the team.
The Occupational Therapy Team in Norwich work with the Social Services Occupational Therapists and have a joint triage and Occupational Therapy waiting list.
