NCHC has a number of multidisciplinary roles within a modern, new build therapy led unit in Norwich opening in 2024.
We are looking for a highly motivated, innovative Band 6 Staff Nurse who is committed to delivering exceptional patient-centred care and join our exciting journey to build our new unit creating a supportive and comfortable environment for 48 patients at NCH&C's Norwich Community Hospital site. Healthcare will be delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and well-being.
Utilisingthe latest therapy and rehabilitation technology and practices, you will be a part of this new team and will play a pivotal role in supportingpatients back into the community and, in this role, we can offer you the opportunity to play a significant part in both the development of the team and therapy led, reablement culture within a brand-new unit.

Main duties of the job
- To deputise for the Nursing Lead as/when appropriate and required.
- To lead on Nursing Practice on the unit to deliver safe, effective, consistent and high-quality admissions, transfer and discharge services for patients and their care givers in a reablement setting.
- To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients and their cares and families.
- To support patients being as independent as possible within their functional limits.
- To support the development of a therapy led, reablement culture within the new unit and to ensure that the ethos becomes part of the culture within the teams.
- Work effectively as a member of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
- To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
- Be responsible for the delivery and co-ordination of care in a reablement setting.
- Work proactively within the team to achieve unit and trust goals and objectives.
- Work with and promote recognised policies, protocols and guidelines.

- Plan and assess programmes of care with realistic nursing goals with the consultation of patient, family and multidisciplinary team.

- Ensure adequate provision and maintenance of equipment
- Assist with the induction of new staff and students, acting as a mentor.
- Act as a role model giving guidance and support to staff and students.
- Participate in problem solving related to work organisation.
