Job summary

NCH&C has a brilliant opportunity for a progressive and forward thinking senior clinician to develop and lead the delivery of reablement and rehabilitation at a new state-of-the-art therapy-led unit in Norwich.

Opening in 2024, we are looking for a highly motivated, innovative person who is committed to delivering patient-centred care and join our exciting journey to build our new unit creating a supportive and comfortable environment for 48 patients at NCH&C's Norwich Community Hospital site. Healthcare will be delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and well-being, and we will be looking to our new Unit Lead to help us create this forward thinking, compassionate and resourceful team.

Utilisingthe latest therapy and rehabilitation technology and practices, this new team will play a pivotal role in supportingpatients back into the community and, in this role, we can offer you the opportunity to play a significant part in both the development of the team and therapy led, reablement culture within a brand new unit.

Main duties of the job

Utilisingthe latest therapy and rehabilitation technology and practices, this new team will play a pivotal role in supportingpatients back into the community and, in this role, we can offer you the opportunity to play a significant part in both the development of the team and therapy led, reablement culture within a brand new unit.

1. The Unit Lead is integral to the delivery of safe, effective, consistent and high-quality admissions, transfer and discharge services for patients and their care givers in a reablement setting.

2. To provide senior leadership and lead on the development of a therapy led, reablement culture within the new unit and to ensure that the ethos becomes part of the culture within the teams.

3. To lead a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.

4. To lead and support the Clinical and Therapy Leads to lead and support the Clinical and Therapy staff, Discharge Coordinators, Activities Co-ordinators, Therapy Assistants and administrator(s).

5. To create an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients and their cares and families.6. Work collaboratively with Health and Social Care professionals to maximise admission avoidance opportunities and support a HomeFirst ethos to discharge patients. Lead on capacity or patient flow issues and the effective implementation of escalation procedures when the whole system or one constituent part of the system is unable to manage the presented demand being placed upon it. The post holder will have advanced clinical knowledge in the prioritisation and management of patient care.
