NCHC has a number of multidisciplinary roles within the new state-of-the-art therapy led unit in Norwich opening in 2024.
We are looking for highly motivated, innovative Physiotherapists who are committed to delivering exceptional patient-centred care and join our exciting journey to build our new unit creating a supportive and comfortable environment for 48 patients at NCH&C's Norwich Community Hospital site. Healthcare will be delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and well-being.
Utilising the latest therapy and rehabilitation technology and practices, you will be a part of this new team and will play a pivotal role in supporting patients back into the community and, in this role, we can offer you the opportunity to play a significant part in both the development of the team and therapy led, reablement culture within a brand new unit.

Main duties of the job
- To contribute to the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, community hospitals and in clinics, by assessing, planning, implementing and evaluating packages of care.
- Be responsible for delivery of care in a reablement setting.
- To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
- To support patients being as independent as possible within their functional limits.
- To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
- To support and deliver innovation within discharge practice and pathways.
- To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
- Be responsible for the delivery and co-ordination of care to a named group of patients.
- Work proactively within the team to achieve unit and trust goals and
- Work with and promote recognised policies, protocols, and guidelines.

Professional/Clinical Responsibilities
- To be wholly accountable for own professional practice as determined by the Health Care Professions Council code of conduct. Take every reasonable opportunity to sustain and improve own knowledge and professional competence. To continue any development highlighted through the appraisal process and attend mandatory and specific training sessions.
- Undertake comprehensive, holistic physiotherapy assessments of patients (including those with a complex presentation) using investigative, analytical and clinical reasoning skills to form a differential diagnosis.
- Plan and assess programmes of care with realistic physiotherapeutic goals with the consultation of the patient, family, multidisciplinary team and other agencies.
- To ensure that informed consent is obtained prior to initiating all interventions and to be respectful of patient needs with regard to privacy, dignity and courtesy.
- Provide information and education to enable patients to make choices regarding a healthier lifestyle, as appropriate.
- Where there are barriers to understanding, such as hearing impairments, mental capacity impairments and other difficulties in comprehension, consider alternative methods to facilitate understanding, consent and concordance.
- Promote effective communication within the multi-disciplinary team, with GPs, Social Services and other outside agencies, including Voluntary services and to work collaboratively with these services, as appropriate.
- Recommend innovative changes in clinical practice to enhance patient care.
- Demonstrate an understanding of Assistive Technology and make referrals for this as required.
