NCHC has a number of multidisciplinary roles within the new state-of-the-art therapy led unit in Norwich opening in 2024.
We are looking for highly motivated, innovative Physiotherapists who are committed to delivering exceptional patient-centred care and join our exciting journey to build our new unit creating a supportive and comfortable environment for 48 patients at NCH&C's Norwich Community Hospital site. Healthcare will be delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and well-being.
Utilising the latest therapy and rehabilitation technology and practices, you will be a part of this new team and will play a pivotal role in supporting patients back into the community and, in this role, we can offer you the opportunity to play a significant part in both the development of the team and therapy led, reablement culture within a brand new unit.

Main duties of the job
To contribute to the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, community hospitals and in clinics, by assessing, planning, implementing and evaluating packages of care.
Be responsible for delivery of care in a reablement setting supporting an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
To support patients being as independent as possible within their functional limits as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
To support and deliver innovation within discharge practice and pathways working with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
Responsible for the delivery and co-ordination of care to a named group of patients, working within the team to achieve unit and trust goals andpromote recognised policies, protocols, and guidelines.
Work as an active member of the Physiotherapy team, along with the other members of the Trusts Multidisciplinary team. This may include liaising with other Trusts, statutory and voluntary organisations e.g. GP's, members of the Biomechanics team, voluntary organisations, carers etc.

Clinical
The post holder will:
- Undertake appropriate Physiotherapy assessments of patients, using investigative, analytical, clinical reasoning skills and outcome measures to inform the development of a graded treatment plan, which may need to be delivered from a range of options.
- Develop, monitor and review individualised physiotherapy treatment plans of patients, in collaboration with the integrated team, other agencies and the patient, utilising a range of treatment techniques & equipment to maximise their functional ability.
- Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
- Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
- Be required to use tact and persuasive skills in order to gain the patients co-operation in their treatment plan.
- Be aware of assistive technology and refer and utilise as required.
- Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
- To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
- Ensure own caseload is planned and prioritised according to service targets and patient need.
- Ensure patient held records are completed for each session/visit/clinic appointment, and that documentation is completed within 24 hours.
- Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual handling and mobilising skills.
- Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
- Ensure informed consent is obtained prior to initiating interventions.
- Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
