An exciting opportunity for an individual looking for a 12 month secondment to a Band 6 Community Link Nurse role in NN3.
We are looking for an experienced registered nurse to join our dedicated and highly professional team of nurses working in Aylsham and the surrounding areas.
This role will offer the opportunity for both clinical and leadership development, whilst working in a caring, friendly and supportive team.
Main duties of the job
You will be responsible for the assessment and treatment of complex patients with a wide variety of health needs of acute or chronic presentation, visiting them within their own homes, care homes, the travelling community and in clinics. We are looking for a nurse who is committed to delivering high standards of care. You will supervise Band 5 Community Nurses and other members of the nursing team, and work closely with the therapy staff to assist the patient to achieve maximal function and independence. In addition, you will support the Clinical Lead Nurse and Clinical Operations Manager with managerial responsibilities.
You will need effective communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service. It is essential to have evidence of professional competencies and have the ability to apply your knowledge and skills to clinical practice. A sound understanding and knowledge of the current drive to transform community services is an essential requirement.
