Are you a nurse, AHP or paramedic looking for a new challenge working within an integrated team?
An exciting Band 7 secondment opportunity has arisen within the Rapid Assessment & Frailty Team who undertake comprehensive admission diversion and frailty assessments within the urgent care areas at the Queen Elizabeth Hospital.
You will have the opportunity to support the clinical leadership of the team, alongside the Team Lead in the delivery of integrated care for patients attending the hospital, working with a number of teams within West Norfolk.
Main duties of the job
Main duties would include;
Acting as the clinical lead within the speciality, providing clinical assessments with patients, providing oversight and expertise for the team.
Providing mentorship, supervision and teaching to team members to enable continued professional development for all staff.
Demonstrate effective and inclusive leadership skills, acting as a clinical role model.
Working alongside the team lead to co-ordinate and develop the service profile, enabling a fit for future care model.

The Rapid Assessment & Frailty Team are dual funded service, provided by both Norfolk Community Health & Care and the Queen Elizabeth Hospital.

To take the role of Clinical Lead for RAFT, working with the Team Lead in the planning, co-ordination and evaluation of the physiotherapy nursing and occupational therapy specialities within the emergency access setting. Providing a Specialist Clinical Lead role for service delivery & development.
Provide specialist clinical knowledge & experience in assessing older, frail patients who often present with multi-pathological problems, including acute confused states, medical/respiratory/neurological or musculo-skeletal conditions.
To respond to a bleep and provide a prompt and effective assessment within agreed standards to ensure the most appropriate and safe discharge to home or other suitable environment.
To ensure the team provide advanced combined physiotherapy, nursing & occupational therapy assessment, diagnosis and treatment of patients within the speciality with an emphasis on those with highly complex and/or chronic presentation.
To carry out home visits and assess and provide specific equipment as required.
To train, educate and support Band 5, Band 6 physiotherapists, nurses, occupational therapists, assistant practitioners, therapy assistants, students and other healthcare professionals.
To share the leadership role with the Norfolk Community Health & Care counterpart to plan, develop, co-ordinate and evaluate the Rapid Assessment Team.
To organise, implement and actively support and take part in the seven day working rota for the integrated Rapid Assessment & Frailty Team.
