Job summary
An exciting opportunity has arisen to join our triage team as a Community Triage Clinician in the North Locality Hub. You will be responsible for triaging incoming referrals assessing the patients' needs, determining the priority of care and processing the referrals accordingly. The position will be approximately 80% triage with the remaining time providing the opportunity to maintain clinical competencies in community nursing teams.

Main duties of the job
- To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
- To document triage notes in order that the patients' record is complete and for audit purposes.
- To use information technology to process the referrals for the 80% triaging role.
- To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
- To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital.
- To work within the integrated team to prevent unnecessary admission to hospital.
- To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
