Job summary

The West Norwich Neighbourhood Community Nursing Team are looking for an enthusiastic full time Band 4 Registered Norsing Associate to join our friendly, supportive teams of nurses, health care assistants, assistant practitioners and other nursing associates. You will be based at Norwich Community Hospital. The West Norwich Team covers the west side of Norwich including Costessy, Bowthorpe, Easton and Taverham.

The Community Nursing service treats patients in their own homes, including residential homes and supported living complexes. We also provide some services in a clinic setting including our leg ulcer clinic and catheter clinic.

Main duties of the job

  1. To provide and monitor safe, person centred care to patients in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.
  2. To support the registered nurse in the contribution to on-going assessment, treatment and care planning for patients
  3. To promote health and prevent ill health in patients and service users and contribute to integrated care
  4. To monitor the condition and health needs of patients in partnership with colleagues, families and carers, referring to others for reassessment, when required
  5. To actively contribute to an effective learning environment and support others in their learning
  6. To adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.

Many of our patients will have complex multiple pathologies and so we aim to provide holistic, patient centred assessments and deliver individualised care.

Full training will be given to gain competency in the clinical skills required, as well as a comprehensive induction period.

You will demonstrate an understanding of the role of community nursing. NCH&C is committed to the development of future health clinicians and there will be an expectation that alongside their clinical responsibilities the Band 4 Registered Nursing Associate will support students and apprentices in the role of supervisor and eventually as an assessor.
