Job summary
To provide clear operational leadership and line management to the intermediate care service. The Clinical Operational Manager will work collaboratively across organisation boundaries to participate in the development of system-wide pathways and to evaluate and monitor the clinical effectiveness and quality of the service.
Main duties of the job
* To manage the Intermediate Care Service
* To facilitate the delivery of service specifications
* To work with discharge services to enable safe discharge and admission avoidance.
* To provide a seamless integrated service.
* To provide line management of the Virtual Ward, Integrated Care Coordinators and Intermediate Care Services. Staff to manage include nurses, HCAs, physiotherapists, OTs, Advanced Practitioners as well as non-clinical staff.
