Intermediate Care Staff Nurse

Job summary

An exciting opportunity has arisen for a band 5 staff nurse to join our Intermediate bed base unit at Padgate house. Working as part of a multidisciplinary team, the post holder will support patients through assessment, treatment, rehabilitation and discharge planning, enabling timely and safe transitions either back to their own homes or to the most appropriate onward care setting. The role requires strong clinical skills, effective communication, and the ability to work flexibly in a fastpaced, recoveryfocused environment.

The post holder will be responsible for assessing, planning, implementing and evaluating evidencebased nursing care, contributing to clinical decisionmaking, and supporting safe patient flow within the unit. They will work collaboratively with therapists, medical staff, social care and community services to ensure coordinated, holistic care and positive patient outcomes.

This post involves rotational shift working, including weekends and bank holidays.

Main duties of the job

Clinical

Perform comprehensive assessment of patient nursing needs.

Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.

Maintain accurate records in accordance with NMC.

Attend a multi-disciplinary case conference as the named nurse if appropriate.

To communicate and collaborate with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care.

To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.

To competently perform clinical skills related to the post.

To be professionally responsible for delegated visits under the supervision of the Intermediate Care Sister / Charge Nurse

To work without direct supervision.

Managerial

To be responsible for work allocations of support staff.

To complete monthly supervisions for support staff.

To be responsible for organising and prioritising own caseload.
