Job summary
To act as a senior clinical
professional within the UCR service, as an integrated health and social care
team providing an urgent rapid response to Warrington residents, experiencing a
health crisis. The aim is to support people to remain at home rather than be
admitted to hospital. Liaising with patients the team and colleagues across the
wider health and social care environment including NWAS the postholder will
promote, maintain and enhance effective multidisciplinary patient care.
To act as an independent
autonomous practitioner to provide specialist treatment and advice to patients
and staff which will contribute to the assessment, diagnosis, care and
treatment of patients at home preventing unnecessary hospital admission. All referrals
will be triaged immediately and responded to within two hours, with referrals
from NWAS prioritised.
Possess and demonstrate
extended clinical skills in the assessment, diagnosis, treatment and management
of a wide range of patient care needs. Professional autonomy and clinical
judgement will be exercised in managing, referring and signposting patients on
to wider community services and assets.
Be responsible for the clinical management of the UCR Team,
ensuring effective and efficient delivery of a high quality timely service to
meet commissioner requirements.
Plan capacity and demand to deliver a safe service and
contibute to admission avoidance and a timely discharge home.
