Job summary
We are seeking a dynamic, highly motivated individual to
become part of our Enhanced Home Care Support Team. The band 6 triage role is
an integral part of the team and will work closely with the nursing and
residential home staff when they are requiring advice, many of whom are frail,
elderly with long term conditions and complex health needs.
Main duties of the job
RGN/RN
Adult Diploma or Degree on the NMC Register
Ability
to work as a team player
The post holder will support the team to undertake triage of the patients
direct from care homes, office advice, support, triage as per Manchester
triage, signpost to a variety of services as required
The post holder will support in the proactive management of patients,
liaising with the named ECHSS nurse for that home to enable management plans
and advance care planning to be in place
Knowledge
of relevant NICE guidelines in relation to clinical practice
The post holder will work under the direction of the clinical lead for
the service and will be responsible for the assessment of patient need.
The post holder will play a part in the mentoring of pre and post
registration students.
