Enhanced Care Home Support Service Triage Nurse

Job summary

We are seeking a dynamic, highly motivated individual to become part of our Enhanced Home Care Support Team. The band 6 triage role is an integral part of the team and will work closely with the nursing and residential home staff when they are requiring advice, many of whom are frail, elderly with long term conditions and complex health needs.

Main duties of the job

RGN/RN Adult Diploma or Degree on the NMC Register

Ability to work as a team player

The post holder will support the team to undertake triage of the patients direct from care homes, office advice, support, triage as per Manchester triage, signpost to a variety of services as required

The post holder will support in the proactive management of patients, liaising with the named ECHSS nurse for that home to enable management plans and advance care planning to be in place

Knowledge of relevant NICE guidelines in relation to clinical practice

The post holder will work under the direction of the clinical lead for the service and will be responsible for the assessment of patient need.

The post holder will play a part in the mentoring of pre and post registration students.

To perform accurate telephone assessment required by health care professionals, social workers, care home staff (clinicians and carers) and patients accessing the service.

Make autonomous clinical decisions to ensure best practice for all patient care.

Provide a high quality triage assessment of all contacts to the triage service. Identify life threatening symptoms and act appropriately.

Provide self care advice and management of presenting symptoms.

Manage patients in relation to their symptoms and clinical history, via care pathways.

Identify treatments as required.

Maintain excellent communications with all team members including care home staff and other Allied Health Care Professionals assuring the development of the MDT for the Enhanced care Home Support Service.

Refer to appropriate services dependant on patient need, to ensure care provided in a timely manner

Maintain confidentiality of all patients identifiable information and contacts.

Maintain clinical knowledge and competence using best practice, evidence based guidelines and supervision.

Maintain skills and competencies in use of the patient call management system.

Inform manager of any professional concerns. Report significant events according to policy.

Management Responsibilities:

Effectively manage the triage and coordination session for the service, planning, prioritising and coordinating the work load across all aspects of the service.

Work in partnership with other clinicians on duty to maintain effective communication about workload, case mix and outcomes.

Liaise with colleagues at other locations to utilise resources effectively and efficiently.

Promote a safe and effective service at all times.

Support other staff as necessary.

Monitor work load, to ensure accredited standards are met through out the session.

Adhere to policy and procedures.

Support and contribute to the future development of the service.

Education and Training Responsibilities:
