Job summary
We are seeking a dynamic, highly motivated individual to
work within the Enhanced Care Home Support Service, to support residents within
a care home setting. You will be required to care for patients living within
both nursing and residential homes, many of whom are frail, elderly with long
term conditions and complex health needs.
The role will involve patient triage and undertaking the clinical
assessment and examination of patients with a view to supporting prompt
diagnosis and treatment of care. The successful candidate will possess the clinical
examination module or will be willing to undertake this module, and non-medical
prescribing would be beneficial. The service works closely with the
multidisciplinary team to deliver the Directive Enhanced Service in
collaboration with the Primary Care Network. The post creates an ideal
opportunity for an experienced nurse wishing to expand their skills and
knowledge within this area of expertise.
Main duties of the job
