Job summary
We are seeking a dynamic,
highly motivated individual to become part of our Enhanced Care Home Support
Service. The advanced practitioner is an integral part of the team, who will be
able to clinically assess, diagnose and prescribe treatment for people within
the nursing and residential homes of Warrington, as well as provide guidance
and support for other staff within the team.
The role will involve visiting
nursing and residential homes to clinically assess patients, making autonomous
clinical decisions based on a history and the clinical presentation of the
person. Enabling a diagnosis and treatment plan to be provided to care home
staff.
The role will include
telephone triage, assessing the patients needs with health care professionals,
social workers, and care home staff, when a patient is unwell, or they are
requiring advice, many of whom are frail, elderly with long term conditions and
complex health needs.
The successful candidate will
work with the multidisciplinary team to deliver the Enhanced Health in Care
Homes Framework in collaboration with the Primary Care Networks.
The post creates an
ideal opportunity for an experienced nurse wishing to expand their skills and
knowledge within this area of expertise. They must be able to demonstrate
excellent communication skills, flexibility, and a commitment to caring for
frail, older people, whilst contributing to the development of the service.
Main duties of the job
The
successful candidate will be a senior clinical nurse within the Enhanced Care
Home Support Service, leading a team and providing both an urgent, reactive
response to people cared for within the nursing and residential homes in
Warrington. To ensure a timely clinical assessment, diagnosis and treatment as
required, as well as proactive, planned holistic assessments, aimed at
optimising management of medical conditions, including frailty, and supporting
with Advance Care Planning. The aim is to support people to remain within the
care home setting rather than admit to hospital, where appropriate.
The role will require
the postholder to act as an independent autonomous practitioner to provide
treatment and advice to patients and care home staff, both face to face and
overthe
telephone, which will contribute to the assessment, diagnosis, care and
treatment of patients, preventing unnecessary hospital admissions.
The postholder must possess and demonstrate extended
clinical skills in the assessment, diagnosis, treatment and management of a
wide range of patient care needs, enabling professional autonomy and clinical
judgement in managing and referring patients on to wider services, whilst
maintaining close working relationships with GPs / Primary Care Networks to
support the Enhanced Health in Care Homes Framework and Multi-Disciplinary Team
(MDT) meetings.
