Job summary

An exciting opportunity has arisen for a Band 6 OPAT Team Leaders. The position will be within the community Warrington & Halton OPAT Service.

The post holder will work closely with the OPAT Lead Nurse to develop and deliver care along agreed clinical pathways both supporting early discharges and preventing hospital admission.

The role will include providing specialized, highly skilled care to patients requiring IV antibiotics or long term vascular access care. You we also be expected to support the OPAT Lead Nurse in delivering support, advice and training to community nurses, secondary care and GPs.

The post provide the opportunity to undertake your V300 NMP qualification.

The core services hours are 7am-8pm, 7 days per week.

Main duties of the job

Principle Responsibilities

Maximise independence by supporting people with conditions requiring IV therapy and highly complex needs to remain in their own home as appropriate, by utilising available resources.

Undertake clinical assessment and provide treatment for patients within a defined group.

Link with existing services to facilitate early supported discharges from hospital and prevent readmission.

Communicate with members of the multi-disciplinary team to ensure care is co-ordinated, safe and effective with the patient at the forefront of the decision making process.

Conduct a comprehensive health assessment, utilising evidence based practice in order to formulate patient centred care plans. This will include:

Health assessment including medical history

Physical examination

Assessment, review and prescribing of medication utilising extended prescribing role.

Requesting diagnostic tests

Social care needs referrals.

Develop, monitor, and manage the plan of care in collaboration with members of the MDT. This will involve

Application of clinical knowledge about IV therapy

Analysis of symptoms and data

Identification of risk factors associated with exacerbation of patients condition.

Recognition of early symptoms of acute illness.

Involving patients and carers in the development of the care plan and ensuring that their views and abilities are reflected.

Documentation of progress and continuous reassessment

Referral and investigations.

Co-ordinate care and treatment to avoid fragmentation, duplication, and delay in treatment. You will be required to

Prioritise and coordinate health needs

Refer to appropriate specialist services

Effectively communicate and share appropriate information with fellow health professional to avoid conflicting treatments.

Provide training, mentor, and preceptorship for community nurses in the assessment of peripheral cannulation and the administration of intravenous antibiotics. Ensuring competences is achieved. .

Participate in individual and group clinical supervision and action learning sets and take responsibility for developing own learning.

Participate in research and audit relating to IV/OPAT therapy

Ensure systems are in place for ongoing review and assessment of care provision and delivery.

Improve quality via clinical governance, by working closely wth colleagues to address competency levels within the service.

Support and contribute to the future development of the service

Report incidents through application of trust policies.

Participate in patient satisfaction reporting to improve patient care.

Manage the complex clinical interventions of individuals within an identified patient groups on an going basis.

Ensure the safe management of care and service delivery

Line manage a defined team of staff, including performing appraisal, personal development reviews and the application of staff management procedures.

Contribute to the achievement of reduced hospital admissions and length of hospital stay.

Promote formal and informal training to both pre and post registration staff.
