Job summary
An exciting opportunity has arisen for a Band 6 OPAT Team Leaders. The
position will be within the community Warrington & Halton OPAT Service.
The post holder will work closely with the OPAT Lead Nurse to develop
and deliver care along agreed clinical pathways both supporting early
discharges and preventing hospital admission.
The role will include providing specialized, highly skilled care to
patients requiring IV antibiotics or long term vascular access care. You we
also be expected to support the OPAT Lead Nurse in delivering support, advice
and training to community nurses, secondary care and GPs.
The post provide the opportunity to undertake your V300 NMP
qualification.
The core services hours are 7am-8pm, 7 days per week.
Main duties of the job
Principle
Responsibilities
Maximise
independence by supporting people with conditions requiring IV therapy and
highly complex needs to remain in their own home as appropriate, by utilising
available resources.
Undertake
clinical assessment and provide treatment for patients within a defined group.
Link with
existing services to facilitate early supported discharges from hospital and
prevent readmission.
Communicate
with members of the multi-disciplinary team to ensure care is co-ordinated,
safe and effective with the patient at the forefront of the decision making
process.
