Job summary
The shift pattern for this vacancy would be 19.30-08.00, 3 nights a week over a 7
day period.
A dynamic dedicated nurse is
required to compliment our current team of staff within our intermediate care
bed base. Providing rehabilitation for both step down and step up patients you
need to have a good all round skill set to support the needs of these patients.
Working as part of a dedicated
integrated health and social care team you will develop a variety of skills. A
foundation of basic nursing skills is required.
Main duties of the job
Ensuring
a high standard of care with intermediate care bed based facility.
Liaises
with the Intermediate Care Sister / Charge Nurse to ensure information in
relation to patient care/changes to care plan and the continuity of care.
Provides
mentoring, teaching and assessing to support staff.
Liaises
with all health care professionals and other agencies to ensure effective
service delivery in the intermediate care facility.
Perform
comprehensive assessment of patient nursing needs.
Assess,
plan, implement and evaluate programmes of care according to changing health
needs under the direction of the Intermediate Care Sister / Charge Nurse.
Maintain
accurate records in accordance with NMC.
Attend
a multi-disciplinary case conference as the named nurse if appropriate.
To
communicate and collaborate with other members of the Integrated Intermediate
Care Team, PHCT, voluntary and statutory organisations to support individual
patient care.
To
establish and maintain effective communication with patients, relatives and
carers in complex and potentially challenging situations.
To
competently perform clinical skills related to the post.
To be
professionally responsible for delegated visits under the supervision of the
Intermediate Care Sister / Charge Nurse
To work without direct supervision.
