Job summary
An opportunity has arisen for a highly motivated individual to join the
community Warrington & Halton OPAT/IV service as an OPAT/IV Nurse.
The post holder will work closely with the OPAT Team leaders to deliver
care along agreed clinical pathways both supporting early discharges and
preventing hospital admission.
The role will include providing specialized, highly skilled care to
patients requiring IV antibiotics or long term vascular access care. You we
also be expected to support the OPAT team leaders in delivering support, advice
and training to community nurses, secondary care and GPs.
The core services hours are 7am-8pm, 7 days per week.
Main duties of the job
Principal Responsibilities
- To perform peripheral cannulation and venepuncture in
accordance with established protocols.

To perform port-a-cath procedures
according to established protocols.

To act as an assessor to district
nurses in the assessment of the administration of intravenous antibiotics.

To provide clinical support and advice
to the patient and district nurses in the absence of the IV Team Leader. e.g.
during holidays, study leave.

To work alongside the district nurse
teams to support them in providing intravenous therapy within the patients
home environment.

To work under the direction of the IV
Team Leader.

To work within Trust policies and
guidelines and follow the intravenous therapy patient care pathways.

To provide a safe, seamless
transition of care by using a multi-disciplinary approach to care.

To work without direct supervision
of the IV Team Leader in the patients own environment and also within a clinic
setting. Reporting to the IV Team Leader.

development, implementation and evaluation of care.

care from the pathway.

communication of the patients progress to IV Team Leader.
