As a Community Nurse within our
019 Healthy Child Programme, you will play a key role in shaping how we
support children and families to thrive. Working as part of an innovative
Family Hub model, you will focus on strengthening the home learning environment,
improving engagement with families and increasing attendance and outcomes from
developmental reviews.
This is a highly collaborative
role, where you will build strong relationships with Specialist community
public health nurses, early years settings, education and wider partners to
ensure children are identified early and supported through the right pathways
at the right time. You will bring curiosity, initiative and a passion for
improving outcomes, helping to reduce inequalities, address missed
opportunities and support more children to achieve a Good Level of Development.

Main duties of the job
In this role, you will take a
proactive, outreach-focused approach to engaging children and families who are
not accessing universal services or early years provision. You will work across
community, home and partner settings to understand barriers to engagement,
using relationship-based practice to build trust and improve attendance at
developmental reviews.
You will play a central role in
strengthening early identification, using professional curiosity, data and
partnership intelligence to identify children who may be at risk of poorer
outcomes. Working closely with early years, education and multidisciplinary
partners, you will support timely assessment, escalation and intervention,
ensuring children are on the most appropriate pathway as early as possible.
A key part of the role will
involve closing gaps in developmental outcomes by targeting those children who
are not routinely seen or whose needs are not yet understood. You will
contribute to improving the quality and coverage of developmental reviews, reducing
missed opportunities and supporting more children to achieve expected levels of
development.
You will also help
shape and embed new ways of working across the Healthy Child Programme and
Family Hub network, contributing to service improvement, pathway development
and collaborative approaches to care.

Job Purpose
As a registered nurse, the post holder will work within the 0-19 integrated public health nursing Team to deliver a flexible 8am-8pm service providing access to the Healthy Child Programme for children, young people and families accessing the 0-19 Service.
The post holder will have delegated responsibility for the assessment of Health needs, implementation and evaluation of care as part of the Healthy Child Programme, at community, Universal, Universal Partnership and Universal Partnership Plus level.
This will include children with complex health needs; children/young people who may be presenting with a cause for concern, subject to safeguarding and Children in Care; parents/young people who are experiencing emotional health needs.
Duties and Responsibilities
- To support delivery of the Healthy Child Programme using evidence based practice to actively promote the health of children and young people.
- To be a named / link nurse for designated schools/ early years settings whilst holding a caseload which incorporates the 4 levels of intervention (Community, Universal, Universal Plus and Universal Partnership Plus).
- To deliver and engage in health promotion programmes and public health initiatives to prevent ill health, early identification of abnormalities and work with all clients in the 0-19 service and communities to reduce health inequalities as identified within individual Health Plans.
- To conduct health needs assessments in the home, school, clinics or community settings in a planned programme of care within a variety of settings.
- Offer appropriate interventions, delivering agreed programmes of care and evaluating the outcomes for the child, young person and family.
- Plan and implement school entry assessments delegating duties as appropriate.
- Plan and implement health assessment at transition from primary to secondary school delegating duties as appropriate.
8. Undertake comprehensive health assessments for identified babies, children and young people, including annual and bi annual health assessments for those in local authority care in line with the Healthy Child Programme for children 0-19 years of age.

from both voluntary and statutory organisations.

To attend Child Protection case conferences and other meetings as appropriate.
- To provide on-going support for children and young people in care including the statutory health assessment and on-going review.

- Feedback the views and needs of children/young people, families and schools to the named SCPHN School Nurse/SCPHN Health Visitor as necessary, to enable appropriate interventions to be implemented.
