Job summary
Bridgewater community health community trust are seeking a dedicated and compassionate Health Visitor to join our dynamic Halton team, working to improve outcomes for children, families, and communities. As a qualified Specialist Community Public Health Nurse (SCPHN), you will deliver the Healthy Child Programme, providing vital support from pregnancy through to school entry.
You will work in partnership with parents, carers, early years providers, and other professionals to promote health and wellbeing, identify health needs early, prevent problems from escalating, and safeguard children. Main duties of the job
Halton is a geographically small area however provides a diverse community, ranging from affluent tosocially deprived. It is an area that provides excellent experiences for Health Visitors, especially those that wish to specialise in specific areas.
The post holder will be responsible for assessing the needs of the local population and delivering health promotion services to promote health & well-being and prevent ill health amongst Children, Young People & Families.
You will work as part of one of the teams in Halton, consisting of Team Leaders, Health Visitors, Community Nurses & Nursery Nurses. This is where you will be supported whilst working with your own individual caseload, as well as in partnership with the newly developed Family Hubs to drive forward the national agenda.
