Halton 0-19 team are proud and excited to be one of the 75 areas who have been awarded additional funds to develop the Family Hub model, working collaboratively with our partners and families to co-produce services to meet the needs of our population.
We are proud to be able to offer additional training in infant mental health, parenting support and infant feeding in our collaborate approach with our partners.

Main duties of the job
Halton is a geographically small area however provides a diverse community, ranging from affluent to socially deprived. It is an area that provides excellent experiences for nurses and the opportunities to develop their public health skills.
You will work as part of one of the 0-19 teams in Halton, consisting of Team Leaders, Health Visitors, School Nurses, Public Health Community Nurses, Public Health Child and Family practitioners Public Health Assistants, and Administrators. This is how you will be supported whilst working with children, young people and families in school, home, the community and clinic setting as well as in partnership with Haltons Family Hubs to drive forward the national agenda. Supporting families to achieve positive outcomes, in line with the Healthy Child Programme.
The post holder will have delegated responsibility for the assessment of health needs, implementation and evaluation of care as part of the Healthy Child Programme, at Universal, Targeted and Specialist level.
This will include children with complex health needs; children/young people who may be presenting with a cause for concern, subject to safeguarding and Children in Care; parents/young people who are experiencing emotional health needs.
