Job summary

An exciting opportunity has arisen to join the community nursing service in Halton. Are you an enthusiastic and motivated individual looking for a new challenge? You will join a team delivering high quality care to patients who are housebound through supporting the team within phlebotomy task, including admin duties, taking referrals, liaising with patients and the wider MDT. Applicants must have excellent interpersonal and communication skills, I.T skills- word, excel and outlook. If you feel you have the right skills to join our team of District nurses, then we would like to hear from you. You must be willing to work within an integrated team and develop your skills and knowledge. Community nursing is evolving at a fast pace, and you must be able to embrace change.

Main duties of the job

Adult venepuncture is performed with referrals, collaborating with team members and promptly reporting patient condition changes. Patients, relatives, and carers receive support, and equality and individual rights are maintained. The role involves service development, administrative tasks, accurate data entry on blood forms, following standard identity checks, and clear communication of procedures to patients. Equipment is safely managed, and records are precisely kept in the IT system. All patients are treated respectfully. Basic clinical tasks are completed as instructed by the district nursing sister. Stock is monitored and reordered via Shared Business Services, and environmental cleanliness is maintained per Infection Control Guidelines.

Job Purpose:

To provide a Adult Phlebotomy Service to patients registered with a Halton GP. This service provides a community based service.

Duties and Responsibilities:

To perform venepuncture techniques to obtain blood from adult population as directed by referral from GPs, Consultants, Nursing staff and other appropriate agencies

To transfer personal and clinical data from referral to completion of correct blood form

To use standard procedures to identify the correct patient. To establish communication with the patient to explain the procedure and allay fears in order to obtain co-operation

To observe and be responsible for the safe use of equipment

Clinical:

Ability to select the most favourable site for venepuncture and to competently perform the procedure

Awareness of the risk involved with this invasive procedure, and the ability to offer advice on aftercare, if required

A degree of medical knowledge is needed in order to minimise discomfort or risk to patients using the service

A knowledge and understanding of the different collection bottles and the correct order when collecting more than one sample, ensuring the correct form is completed

A knowledge of special requirements when collecting specific bloods such as bloods for genetic testing and fasting sugars

Maintenance of a clean and safe environment in the working area at all times.

Ability to recognise and summon help in emergencies

Transportation of samples to collection points in accordance with Health & Safety procedures

To monitor and maintain stock at all times

A knowledge of infection control and the disposal of equipment in accordance with Health & Safety procedures

Awareness of all relevant Trust policies and procedures

Communication:

Ability to communicate with a range of agencies overcoming communication problems with people who may have various conditions, including learning difficulties, sensory difficulties, acute illness, complex needs, life threatening and life limiting conditions

Ability to tactfully deal with behaviour issues, whilst evaluating the reasons for the behaviour and offering guidance e.g. where to obtain appropriate help i.e GP, Practice Nurse, Health Visitor, Childrens Centres etc.

Ability to communicate with GPs, Out-Patient Departments and Paediatric Consultants

Ability to liaise with multidisciplinary teams, parents, carers and family members regarding access to/and security of patient

Ability to communicate with patient/family and ensure they understand the procedure and gain verbal consent in accordance with Trust policy

Managerial/Leadership

To work unsupervised, organise own work schedule, plan appropriate route to meet the demands of the service

To maintain a communication book and appointment diary for each base, prioritising requests when necessary to ensure the service runs efficiently

To distribute work to other bases via email or telephone

To resolve, where possible, any complaints or problems encountered by people using the service

To have knowledge of the referral criteria and inform the requesting agency if a patient is not eligible. To refer patients back to the requesting agency in cases of non-compliance or where the procedure is no longer appropriate/needed

Leadership:

Self-management, being aware of whom to report incidents to. Responsibility for own area of work, but flexible in other areas

Education and Development

Demonstrate phlebotomy procedure to new staff and other health care workers

Participate in updates as and when required

Participate in induction programmes for associated nursing team, students and staff in partner agencies.

Ensure own compliance with regard to mandatory training and PREP requirements.

Clinical Governance

Participate in research, clinical audit, benchmarking and clinical trials in order to improve effectiveness of patient care.

Promote patient and public involvement leading to service improvement.
