Job summary

This position offers an excellent opportunity for an Associate Nurse to become part of our district Nursing team in Halton. In this role, the Nursing Associate will collaborate with Registered Nurses and other members of the multidisciplinary team to provide and monitor care, supporting ongoing assessments under the guidance of supervising clinicians and according to service pathways. They will also work alongside the team to regularly observe and respond to the health needs of those in their care.

Main duties of the job

The role of the Nursing Associate is to work with the Registered Nurse and other members of the multi professional team, providing and monitoring care and contributing to the ongoing assessment, as agreed with the supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team

All concerns and queries would be escalated to the Registered Nurse on duty who will provide advice and guidance. You will communicate clearly through a variety of channels with a broad spectrum of individualsincluding the public, health professionals, and social care workersalways keeping the focus on enhancing and delivering health and care services.

You will show strong interpersonal abilities that foster understanding, compassion, empathy, respect, and trust while regularly interacting with patients, staff, and visitors.

As part of this role, you will share information received from individuals, carers, and colleagues with the appropriate registered care professionals to support optimal care and encourage multidisciplinary or partnership-based approaches.

Deliver compassionate, high-quality care under the supervision of a registered nurse or other qualified professional, emphasizing health promotion and independence.

You will carry out delegated clinical tasks to a high standard, ensuring excellent patient care and effective partnership working.

You will work independently as needed, follow the patient's care plan within your Nursing Associate scope, seek clinical advice when necessary, and keep the team informed of progress.

Support registered nurses with ongoing assessment, planning, management, and care evaluation while maintaining a thorough understanding of the nursing process.

Provide comprehensive support for all aspects of personal care, utilising person-centred approaches to promote autonomy and well-being. This includes initiatives such as Making Every Contact Count.

Perform duties relevant to specialties like venepuncture, and medication administration in accordance with trust policy.

Conduct and document clinical observationssuch as blood pressure, temperature, respiration rate, pulse, peak flow, and oximetryalongside other speciality-specific requirements.

Accurately record nutritional and fluid intake to detect dehydration or fluid retention, raising concerns when identified.

Utilize up-to-date assessment tools, such as Purpose T scores and nutrition assessments.

Measure and interpret blood glucose levels, respond to emergency situations, administer basic first aid (including BLS), and monitor comfort, pain, and rest patterns.

Apply appropriate positioning, pressure-relief techniques, and use manual handling equipment safely.

Monitor wound healing and perform wound care according to evidence-based practices, including aseptic non-touch technique and catheter management and documentation for all genders.

Help implement actions that meet the physical, emotional, psychological, social, cultural, and spiritual needs of patients and their families.

Support registered nurses with complex care, as deemed appropriate.

Protect the privacy, dignity, and safety of each individual in your care.

Ensure that all measures are taken to safeguard patients property and belongings.

Recognize safeguarding issues concerning vulnerable children and adults, reporting or escalating issues promptly to the correct professionals.

Assist in implementing and monitoring clinical standards and outcome measures.
