Job summary
We
are looking for an enthusiastic band 6 District Nurse team leaders to deliver
care to the housebound population within the primary care setting. You will be
caring for patients with long term conditions and complex health needs,
developing your skills and knowledge, whilst working towards an integrated
healthcare structure. You should hold the Specialist Practitioner Degree
qualification (SPQ). You will be responsible for leading teams and liaising
with stakeholders ensuring the delivery of safe effective care. You should be
able to demonstrate you can make clinical decisions to provide evidence-based
practice and care planning in response to clinical assessments.
Main duties of the job
The
successful candidate will perform holistic assessments of patient nursing
needs. Plan, assess, implement and evaluate care delivery according to changing
health needs utilising prescribing skills.
Support
the care management of complex/continuing care packages.
Ensure
practice is evidence based through undertaking audit and critically appraising
literature to achieve a high standard of care.
To
establish and maintain effective communication with stake holders, individual
groups and communities in complex and potentially challenging situations.
Involve
patients and carers in the planning and delivery of care.
To
competently perform a range of clinical skills related to the post.
Maintain
accurate records in accordance with NMC Standards.
To
participate in the health needs assessment of the practice population to
identify areas of unmet need.
Ability
to work without direct supervision and to solve problems.
Undertake
chronic disease management reviews of the housebound patient using appropriate
template
Work
on a rota basis to facilitate weekend working across the district nursing teams
